To apply for assistance from RCCS, complete an application and ask your rabbi to complete his affidavit regarding your eligibility. Return them to the RCCS office.
Your application will be reviewed by our medical review board, and someone will contact you when they have made a decision.
Both forms must be filled out completely. Incomplete applications will delay the application procedure.
